1. PARA SA MGA MAGTATRABAHO
Employer po ninyo ang mag-aaply or mag-update sa inyong PhilHealth number.
a. Mag fill-out lamang ng form na PhilHealth Member Registration Form (PMRF). Maaari itong idownload sa link na ito: https://www.philhealth.gov.ph/downloads/membership/pmrf_012020.pdf

b. Ipasa ito sa inyong employer o inaaplayan kasama ng inyong documentary requirements (e.g. Kopya ng Birth certificate or marriage contract. Kung sakaling walang birth certificate o marriage contract ay mangyaring bisitahin ang aming listahan ng iba pang documentary requirements:

2. PARA SA MGA ESTUDYANTE NA 20 YEARS OLD AND BELOW NA GAGAMITIN SA SCHOOL
Ibigay lamang ang MDR ng inyong magulang na nakadeklara ang inyong pangalan bilang kanilang dependent.


3. PARA SA MGA KUKUHA NG KOPYA NG MDR AT KONTRIBUSYON
Ito ay maaari ninyong idownload sa aming website. Gumawa lamang ng account sa: https://memberinquiry.philhealth.gov.ph/member

4. PARA SA MGA KUKUHA NG PHILHEALTH IDENTIFICATION CARD
  • Magdala ng 1pc latest 1x1 picture
  • Magdala ng kopya ng any valid ID o katunayan ng pagkakakilanlan.
